2008 Student-Athletes Encouraged To Fill Out Medical Paperwork

All student-athletes that want to participate in athletics for the 2008-09 school year need to go on-line to fill out their medical paperwork as soon as possible. To eliminate any confusion prior to your season, please have your paperwork filled out prior to your arrival on campus, to prevent any delays in your ability to practice.

You can fill out the paperwork at this location. You have to fill out the form on-line, print it and send it back to the athletic training staff.
